The most recent versions of official psychiatric diagnostic guidelines include a new addition: Prolonged Grief Disorder (PGD). PGD is controversial due to concerns about harmful looping effects. Some opponents of PGD’s inclusion in the DSM worry that the diagnosis may pathologize normal human experiences and alienate grievers from their grief. This paper argues that these concerns are less troubling than they initially appear (in part because they assume an unhelpful, and conceptually optional, background understanding of health conditions as pathologies) and (...) calls attention to overlooked beneficial looping effects that might be achieved by medicalizing (some) experiences of grief. Nicholas Makins makes a valuable contribution to the literature on medical decision-making, highlighting the role that risk attitudes play in deliberation and subsequently arguing that, in medical choices under uncertainty, if considerations of autonomy and beneficence support deference to patient values and outcome preferences then they also support deference to patients’ attitudes to risk.1 Crucially, however, Makins suggests that it is not simply first-order risk attitudes that are the appropriate target of deference but, rather, patients’ higher-order risk attitudes. In other (...) words, Makins argues that if considerations of autonomy and beneficence support deference in medical decision-making, this deference should include the risk attitudes that individuals desire to have or reflectively endorse. This commentary draws out a morally relevant and overlooked distinction between: (1) deferring to patients’ higher-order attitudes that endorse the first-order risk attitudes they currently hold. and (2) deferring to patients’ higher-order attitudes that endorse risk attitudes that are at odds with the first-order risk attitudes they currently hold (which equates to a higher-order desire to change one’s first-order risk-attitudes). I agree with Makins that if respect for autonomy and beneficence support deference to patient preferences, then these considerations may also support deference to at least some higher-order risk attitudes, namely, those of type (1). Disproportional morbidity and mortality experienced by ethnic minorities in the UK have been highlighted by the COVID-19 pandemic. The ‘Black Lives Matter’ movement has exposed structural racism’s contribution to these health inequities. ‘Cultural Safety’, an antiracist, decolonising and educational innovation originating in New Zealand, has been adopted in Australia. Cultural Safety aims to dismantle barriers faced by colonised Indigenous peoples in mainstream healthcare by addressing systemic racism.This paper explores what it means to be ‘culturally safe’. The ways in which New (...) Zealand and Australia are incorporating Cultural Safety into educating healthcare professionals and in day-to-day practice in medicine are highlighted. We consider the ‘nuts and bolts’ of translating Cultural Safety into the UK to reduce racism within healthcare. Listening to the voices of black, Asian and minority ethnic National Health Service (NHS) consumers, education in reflexivity, both personal and organisational within the NHS are key. By listening to Indigenous colonised peoples, the ex-Empire may find solutions to health inequity. A decolonising feedback loop is required; however, we should take care not to culturally appropriate this valuable reverse innovation. (shrink)

Interpreting the letters characterized as written by Alice Alington and Margaret Roper in 1534 has proved perplexing since their first publication, when the editor wrote, “It is not certainly known” whether Thomas More or Roper wrote the letter to Alington. Did Roper, More, or both write it? This study looks at both letters from a variety of perspectives, pointing out many reasons that complicate reading them before focusing on the personal and political circumstances, the structural knot of wise/foolish, and (...) the writing styles of father and daughter, including an analysis of Roper's known writing, characteristically empathic. It agrees that More was the chief writer, but that Roper might well have written some, though not all, of her speeches, and that she was involved in their discussions and as More's personal representative. She hoped to sell select pieces from her famed (...) wardrobe in order to supplement her income, which was, at the time, rather meager. Accompanying the recently widowed socialite was her self-described “modiste” and “confidante ” Elizabeth Keckley, a formerly enslaved African American dressmaker who had been “intimately associated” with Lincoln for years.1 Despite the various measures undertaken to ensure the scheme’s success, including attempts to extort monied East Coast Republicans and strategic appeals to the press, it ultimately proved fruitless. Political nemeses seized on the opportunity to publicly mock Lincoln, who was seen as an “extravagant and improper” social climber, for her “pecuniary embarrassment,” treating the public auction of her clothing as a spectacle akin to the World Fair.2 Eventually, the former First Lady was forced to retreat to her home in Chicago, leaving Keckley behind to tend to her affairs. 1. However, perhaps unsurprisingly, the book’s release had the exact opposite effect: Keckley’s literary disclosures were met with scorn, regarded by large swaths of white Americans, including some of the same people who had been most vocal in their condemnation of Mary Todd Lincoln, as constituting a set of socially impermissible transgressions of class, race, and gender. Sales of the book were subsequently suppressed, Keckley’s relationship with Lincoln (who had been caught off guard by the text’s release) came to an abrupt end, and there was even a brief, parodic pamphlet produced, titled Behind the Seams; by a Nigger Woman Who Took in Work from Mrs. Lincoln and Mrs. Davis, which cast the entire ordeal as a sort of (racist) cautionary tale.5 Given the clear disdain held for Lincoln by people across the political spectrum at the time, it can be difficult to make sense of the visceral reaction prompted by Keckley’s airing of the controversial figure’s dirty laundry (pun intended). Considering the ways Behind the Scenes implicitly challenged the fixity of hierarchical distinctions between Black and white, employee and employer, however, repudiation of the work might be understood as inevitable. One of Chicago's great cultural achievements, the Institute of Design was among the most important schools of photography in twentieth-century America. It began as an outpost of experimental Bauhaus education and was home to an astonishing group of influential teachers and students, including Lázló Moholy-Nagy, Harry Callahan, and Aaron Siskind. To date, however, the ID's enormous contributions to the art and practice of photography have gone largely unexplored. Research on human infants has begun to shed light on early-developing processes for segmenting perceptual arrays into objects. Infants appear to perceive objects by analyzing three-dimensional surface arrangements and motions. Their perception does not accord with a general tendency to maximize figural goodness or to attend to nonaccidental geometric relations in visual arrays. Object perception does accord with principles governing the motions of material bodies: Infants divide perceptual arrays into units that move as connected wholes, that move separately from one (...) another, that tend to maintain their size and shape over motion, and that tend to act upon each other only on contact. These findings suggest that o general representation of object unity and boundaries is interposed between representations of surfaces and representations of objects of familiar kinds. The processes that construct this representation may be related to processes of physical reasoning. (shrink)

The concept of vulnerability is widely used in bioethics, particularly in research ethics and public health ethics. The traditional approach construes vulnerability as inherent in individuals or the groups to which they belong and views vulnerability as requiring special protections. Florencia Luna and other bioethicists continue to challenge traditional ways of conceptualizing and applying the term. Luna began proposing a layered approach to this concept and recently extended this proposal to offer two new concepts to analyze the concept of vulnerability, (...) namely understanding external conditions that trigger vulnerability and layers of vulnerability with cascading effects. Luna's conception of vulnerability is useful, which we demonstrate by applying her layered view and the new analyses in multiple contexts. We begin by outlining Luna's view and we use vignettes from healthcare involving transgender patients, the care of patients in psychiatric contexts, and research involving prisoners to illustrate how each part of Luna's concept elucidates important moral issues. What exactly does it mean to “trust your gut”? What use are gut-level insights when a person is attempting to consciously and deliberately navigate life altering decisions, such as those surrounding marriage, divorce, job changes, home buying, etc.? This essay provides a partial answer to those questions by leveraging the account of “emotional bodily feelings” offered in Tom Cochrane’s The Emotional Mind: A Control Theory of Affective States. This essay shows why “trusting your gut” is a reasonably good path to (...) emotional self-awareness in decision-making. The first half of the essay offers an analysis of what many people mean by the colloquial advice to “trust your gut,” at least when they deploy that advice in context of an ongoing dialogue about a friend or family member’s major life-decision. In the second half, this article briefly summarizes Cochrane’s account of emotions and emotional bodily feelings, and spells out how these concepts might be deployed to understand the advice to “trust your gut” as nearly tantamount to saying “pursue emotional self-awareness.”. Experiments with young infants provide evidence for early-developing capacities to represent physical objects and to reason about object motion. Early physical reasoning accords with 2 constraints at the center of mature physical conceptions: continuity and solidity. It fails to accord with 2 constraints that may be peripheral to mature conceptions: gravity and inertia. These experiments suggest that cognition develops concurrently with perception and action and that development leads to the enrichment of conceptions around an unchanging core. We present an investigation into the relation between design princi- ples in Japanese gardens, and their associated perceptual effects. This leads to the realization that a set of design principles described in a Japanese gardening text by Shingen (1466), shows many parallels to the visual effects of perceptual grouping, studied by the Gestalt school of psychology. Guidelines for composition of rock clusters closely relate to perception of visual figure. Garden design elements are arranged into patterns that simplify figure-ground segmentation, while (...) seemingly balancing the visual salience of subparts and the global arrangement. Visual ‘ground’ is analyzed via medial axis transformation (MAT), often associated with shape perception in humans. MAT analysis reveals implicit structure in the visual ground of a quintessential rock garden design. The MAT structure enables formal com- parison of structure of figure and ground. They share some aesthetic qualities, with interesting differences. Both contain naturalistic asymmetric, self-similar, branching structures. While the branching pattern of the ground converges towards the viewer, that of the figure converges in the opposite direction. (shrink)

This paper argues that an agent’s moral obligations are necessarily connected to her desires. In doing so I will demonstrate that such a view is less revisionary—and more in line with our common-sense views on morality—than philosophers have previously taken it to be. You can hold a desire-based view of moral normativity, I argue, without being (e.g.) a moral relativist or error theorist about morality. I’ll make this argument by showing how two important features of an objective morality are compatible (...) with such a desire-based account: 1) morality’s authoritative nature, 2) our ability to condemn immoral agents. At its core, nonideal theory is an attempt not only to address issues of justice, but it also provides us a lens through which we can articulate our limitations as knowers and reasoners, the ways in which we are relational in our autonomy needs, and the ways in which we are deeply dependent upon institutions and social supports for our agency and identities. Bringing this lens into bioethics means shifting our orientation in our scholarship and our practice. This shift will (...) have implications in how bioethical evaluations are taken up and played out in policies, institutional structures that inform the clinical encounter, and avenues for protection and redress for marginalized and vulnerable populations. It will also allow theorists and researchers to interrogate the status quo, revealing how many standard policies and practices are embedded in social and institutional arrangements that privilege the few or are built on exclusionary norms. The path forward, and the aim of this volume, is to extend the scholarship of nonideal approaches to bioethics. The volume is divided into two main parts. The first is focused on philosophically unpacking nonideal theory as an approach in bioethics. The second offers applications of nonideal theory in environmental ethics, healthcare ethics, public heath ethics, and genetic ethics. Our collective aim is to expand what has been considered nonideal bioethics. The history of nonideal theory has as its point of reference a turn away from Rawlsian justice, but the future of nonideal theory is ripe with possibilities. (shrink)

Although there has been significant attention in clinical ethics to when physicians should follow a parent’s wishes, there has been much less discussion of the obligation to solicit viewpoints and decisions from all caregivers who have equal moral and legal standing in relation to a pediatric patient. How should healthcare professionals respond when one caregiver dominates decision making? We present a case that highlights how these problems played out in an ethical bargain. Ethical bargaining occurs when the parties involved choose (...) not to pursue a morally preferable option for the sake of coming to a resolution. This case is not one of parental disagreement; rather, the medical team agreed to exclude the patient’s mother from decision making if the patient’s father promised to bring their son back to the hospital for necessary medical tests. We argue that there is an obligation to notice and acknowledge power asymmetries in the family unit, which, in this case, was manifested as the marginalization of the female decision maker by the male decision maker. In these scenarios, clinicians should be careful to avoid treating parents as one homogenous unit, and they should take action to enable caregivers’ autonomy and voice. While there are moral and practical limits to how and when physicians should intervene in family dynamics, we discuss the steps that the medical team should have taken in this case to avoid undermining the parental authority of the mother. We conclude by offering recommendations to address and enable caregivers’ autonomy at an institutional level, and we discuss the importance of tracking and responding to damaging family dynamics to prevent ethically impermissible bargaining. The Health (Regulation of Termination of Pregnancy) Act 2018 was commenced on 01/01/2019 in Ireland. The Act provides for legal termination of pregnancy under defined circumstances including for any reason at < 12 weeks gestation; and where two doctors agree there is ‘a condition affecting the foetus that is likely to lead to the death of the foetus either before, or within 28 days of, birth’. As such, abortion for congenital anomaly (CA) can occur at a number of time points, (...) depending on the adjudged severity. Infants born with CAs frequently require significant medical intervention and account for a high proportion of admissions to paediatric intensive care units (PICUs). The purpose of this paper was to evaluate the number of infants with CAs admitted to an Irish PICU in the period before and after the implementation of the Act. All PICU admissions < 1 month of age to a single Irish paediatric hospital between 2012 and 2021 were analysed. CAs were recorded, and the periods before and after the commencement of the Act compared. We found a difference in admissions involving CAs, particularly those related to congenital heart disease involving single ventricle anatomy. It is plausible that this difference was as a result of improved access to abortion services following the implementation of the Act. This article explores the legal conditions related to the abortion of a foetus with a CA in Ireland, and the possible impact of the Independent Review of the Operation of the Health ( Regulation of Termination of Pregnancy) Act 2018. This article has four aims. First, it will consider explicitly, and polemically, the hierarchical relationship between conversation analysis and membership categorization analysis. Whilst the CA ‘juggernaut’ flourishes, the MCA ‘milk float’ is in danger of being run off the road. For MCA to survive either as a separate discipline, or within CA as a focus equivalent to other ‘generic orders of conversation’, I suggest it must generate new types of systematic studies and reveal fundamental categorial practices. With such a goal (...) in mind, the second aim of the article is to provide a set of clear analytic steps and procedures for conducting MCA, which are grounded in basic categorial and sequential concerns. Third, the article aims to demonstrate how order can be found in the intuitively ‘messy’ discourse phenomenon of membership categories, and how to approach their analysis systematically as a robust feature of particular action-oriented environments. Through the exemplar analyses, the final aim of the article is to promote MCA as a method for interrogating culture, reality and society, without recourse to its reputed ‘wild and promiscuous’ analytic approach. Objective moral facts are supposed to be independent from us, but it has proven difficult to provide a clear account of this independence condition. Objective moral facts cannot be overly independent of us, as even an objective morality would depend, in important respects, on features of us. The challenge is to respect these moral mind-dependencies without inappropriately counting too many moral facts as objective. In this paper, I delineate and evaluate several different versions of the independence condition in moral objectivity. (...) I raise problems for these ways of formulating moral objectivity and then develop a better account of moral objectivity, one that avoids the pitfalls of other proposals. (shrink)

In “Coordination in interpersonal systems,” Emily Butler urges psychologists to move beyond a focus on the individual to better understand dynamic interpersonal systems. She argues that an improved understanding of coordination, in particular, will allow them to not only better understand human behaviour but also solve many social problems, especially polarisation. I agree with both this empirical shift and Butler's normative interest. This said, Butler's framework would benefit from more attention to social identity – which tends to structure polarisation – (...) and emotion – which fuels it. Butler's framework would also be enriched by incorporating knowledge accumulated by political scientists on the import of institutions, the history of party coalitions, and the pursuit of power by social actors. With respect to a reform agenda, a possible place to start is lessening partisan polarisation and thereby strengthening democratic governance, which, at its best, represents an inclusive and synergistic system of human interaction.
